T-Shirt Design

I am looking for an experienced T-Shirt Designer who has worked with Made to Print projects. What I am looking for? I am in the skincare industry and specialize in Acne. I want to create Tshirts that work in different environments that will get people to click the QR code on the back of my tshirt. For example, maybe a Tshirt that says "Got Acne?" in the front and "Scan to join our FREE iBeatAcne Bootcamp" QR Code in the back... when at the Gym? "Are your Gains causing ACNE Pains?" then same call to action in the back... looking for someone that is creative and can design these with the right sizing to be shown on tshirts...
