We are hiring immediately for a reliable, empathetic, and tech-savvy Remote Medical Assistant to support patients enrolled in Chronic Care Management, Behavioral Health Integration, and Remote Patient Monitoring programs.
This role is ideal for someone with strong communication skills, prior clinical knowledge, and the ability to speak with patients in a warm, professional, and supportive manner. The Medical Assistant will conduct monthly patient outreach calls, review patient needs, document care updates, identify barriers to care, provide appropriate education, and help connect patients with community resources.
Responsibilities include completing monthly CCM/BHI/RPM care calls, reviewing patient-reported symptoms and vitals, documenting clearly in the EHR, assisting with medication adherence reminders, identifying social needs such as transportation, food, medication cost, or appointment barriers, and escalating concerns to the physician, lead nurse, or care manager when appropriate.
The ideal candidate must have excellent clinical judgment, understand when a patient concern requires escalation, be comfortable using EHR systems and remote work platforms, and be able to manage multiple patients while staying organized and detail-oriented.
Requirements:
Prior medical assistant or healthcare experience preferred.
Experience with EHR systems preferred.
Strong English communication skills required.
Must be reliable, professional, empathetic, and comfortable speaking with patients by phone.
Must be tech-savvy and able to work independently in a remote setting.
Must be able to start immediately.
Must have stable internet, a quiet workspace, and strong documentation skills.
