Job Overview
TELA Solutions, LLC is seeking a Remote Nurse to conduct comprehensive chart reviews, analyze patient vital signs, and support chronic care management (CCM) services for patients enrolled in our Remote Physiologic Monitoring (RPM) program. This role is crucial in ensuring timely interventions, care coordination, and patient education for individuals managing chronic conditions such as hypertension, diabetes, CHF, and COPD.
Key Responsibilities
• Chart Review & Data Interpretation: Analyze patient histories, RPM data trends, and provider notes to identify clinical concerns.
• Vital Sign Monitoring & Assessment: Evaluate remote physiologic data (BP, glucose, heart rate, weight, SpO2) to detect abnormal trends and determine appropriate follow-up actions.
• Chronic Care Management (CCM): Engage patients in structured monthly care plan discussions, reinforcing disease management strategies, medication adherence, and lifestyle modifications.
• Care Coordination: Collaborate with providers, pharmacists, and other healthcare professionals to address medication adjustments, follow-ups, and referrals as needed.
• Patient Education & Engagement: Provide guidance on symptom management, early warning signs, and preventive measures tailored to each patient's condition.
• Accurate Documentation: Maintain detailed, compliant records of all patient interactions, interventions, and assessments in the EHR.
• Proactive Communication: Reach out to patients regularly to ensure adherence to care plans and encourage engagement in their health management.
Qualifications
• Registered Nurse (RN) or Licensed Practical Nurse (LPN/LVN) with active U.S. licensure (multi-state license preferred).
• Minimum 2 years of clinical experience (preferred in chronic care management, primary care, case management, or telehealth).
• Strong knowledge of chronic disease management, medication interactions, and remote patient monitoring.
• Experience with EHR systems, documentation standards, and HIPAA compliance.
• Ability to interpret remote patient data and apply clinical judgment for proactive care planning.
• C2 English Proficiency Required – candidates must submit a video interview demonstrating fluency in spoken English, clinical terminology, and patient communication.
• Excellent communication, patient education, and care coordination skills.
• Comfortable working in a fast-paced, technology-driven healthcare environment.
Schedule & Compensation
• Monday–Friday, 8-hour daytime shifts (CST)
