Full-Stack Developer Needed to Debug & Finalize Web Form (PHP, AJAX, jQuery)

We have a web form that needs debugging and final improvements. It’s built with PHP (Apache server), jQuery, and AJAX, handling user authentication and form submissions. Issues that need to be fixed primarily revolve around UI.

Requirements:
